If you cannot attend, simply mail your registration using the following form(s).

We offer 4 divisions (players born in 1994 or before are NOT eligible).

To guarantee your child's spot please register on time! Registrations will be taken on a first come, first served basis.

Suggested ages are based on past groupings. Players can play up, but not down by more than 1 year.

REQUIRED EQUIPMENT:
Helmet with full cage (HECC certified only NO MYLECS!!!)
Mouthpiece, elbow pads, hockey gloves, shin guards, skates and a hockey stick Amity Sports will give a 10% discount to all club members
