Time to register for the Spring 2008 Roller Hockey season

Registration will be at Amity Sports on Thursday, March 6, 2008, from 6:30 to 8:00 PM

Bring a new player into the league and receive 50% of your registration fee!
Bring 2 new players into the league and your registration is free!!
New players cannot have played in the league before and require copy of birth certificate.

Age groups are determined by your childŐs birth year, not their age at the time of registration Players born in 1992 or before are not eligible
Below are the age groups for the Spring season (subject to change based on # of players registered):

4/5/6 year old Clinic (2002 +) $75.00
7/8/9 year old Division (1999 - 2001) $75.00 (reduced from $100)
10/11/12 year old Division (1996 - 1998) $100.00
13/14/15 year old Division (1993 - 1995) $100.00

If you are unable to attend, simply mail your registration using the form/address on the reverse side
Please have your registration forms and checks in the mail by Friday, March 7th, 2008
Above special offer does NOT apply to mail in registrations.

Checks should be made payable to John J. McMahon Jr. Memorial Roller Hockey Club

REQUIRED EQUIPMENT:
Helmet with full cage (HECC certified only NO MYLECS!!!)
Mouthpiece, elbow pads, hockey gloves, shin guards, skates and a hockey stick
Amity Sports will give a 10% discount to all club members
